FAQ

Q:    What information is needed for my appointment?
  • Office notes or a referral letter from your referring doctor indicating why you are being referred for neurological evaluation.
  • Bring all diagnostic test results related to your medical problem including CT scans, MRI’s, EEG or EMG.
  • Your medical history including the names of the previously seen neurologist if any.

Q:    Do I need to arrive earlier than my appointment time?
  • If this is your initial visit to our practice, please arrive 15 minutes prior to your scheduled appointment time to allow registration.
  • For a return visit, please notify us if there is a change in insurance carrier, address or telephone number.
Q:    Do I need a referral to see a neurologist?
  • Yes.  If your insurance plan requires a referral from a primary care physician, it’s your responsibility to have it faxed to us at least 24 hours prior to your scheduled appointment.  Please note that a referral from your medical professional is different than an authorization from your insurance company.
